How to Apply to the Traditional BSN
If UH students meet the admission criteria, then they are eligible to apply to the Traditional BSN. Interested students will need to complete the following forms:
The above forms along with the applicant’s ATI TEAS Individual Performance Profile results need to be emailed to nursing@uh.edu.
Applicants will also need to complete the Release of Social Security Number Form
Which can be faxed (832-842-8363) or hand delivered to the Gessner College of Nursing suite (room 306) at the UH Katy campus.
All four documents (Application to Change Course of Study to Nursing Major, ATI TEAS Individual Performance Profile results, Release of Social Security Number Form, and the Course Requirement Checklist) need to be submitted for a student’s application to considered complete.
